Caregiver
The Caregiver will assist residents with their activities of daily living and provide other support duties as directed by the Administrator or Nurse. -Must be able to assist residents with transfers. Must be able to stand for extended periods of time. -Must be able to assist and supervise residents with their activities of daily living to include transferring, bathing, eating, dressing, grooming, medication assistance, and toileting. Must monitor residents' physical and health care needs and document in the communication log daily, notify the administrator and nurse of all concerns. Must have basic computer skills for word processing. Must receive and direct visitors professionally and courteously within appropriate hospitality guidelines. Must be able to prioritize activities and perform multiple tasks simultaneously. Must have communication skills in the predominant language of the property. Must demonstrate competence in serving the needs of aged and chronically ill residents. -High school diploma or equivalent. -Full-time, part-time, or temporary. Days, Evenings, and Nights. -Duties will require movement throughout the building and grounds.

Resident Care Coordinator
Job duties for the RCC are as follows: Filing and file management. Devise an orderly, consistent system for the files currently on hand. Keep resident files in order and updated. Create new resident files as necessary. Changes in the structure of files should be applied to ALL files in order to maintain consistency. Keep current on photographs of residents. A photo of the resident should be placed on the front cover of their file, and in the MOR. -Maintain resident demographics with regards to change of third party providers. Complete and maintain 1823s. Prepare and maintain 911 folders. Answer questions posed by family representatives, health care surrogates, power of attorneys, etc., as appropriate. Prepare for doctor visits (prepare resident files, copies of MORs, etc., and coordinate the movement of patients into the office). -Oversee all medications in the facility for all residents. Daily oversight of the Medication Observation Record (MOR), ensuring medication is properly stored, prepared as scheduled and taken to the resident and then properly documented. Any errors need to be immediately addressed according to protocol. Maintain proper inventory. Ensure that there is a doctor's order for any d/c's or changes of dosage in the MOR. -Directly supervise med techs. Complete elopement evaluations and fall risk evaluations on all residents quarterly. -Nursing degree preferred.
